What is The Foundation For Community-Based Care, Inc.?

This non-profit foundation was established in 1998, for the primary purpose of research and education in the field of in-home and community-based services.

In-home and community based services are perhaps the fastest growing segment of the health care industry. Despite the growth, there is little data available on issues such as cost-effectiveness of in-home services, quality of care and related outcomes, patient satisfaction, and the in-home service labor pool. Therefore, in an effort to reduce this data deficit and enhance community awareness of in-home and community-based options, our non-profit Foundation’s purpose is to conduct health service and workforce improvement studies focusing on in-home and community based health care programs and to help educate consumers, policy makers, and the public at-large.

What are the goals of The Foundation For Community-Based Care, Inc.?

The Foundation’s goals are as follows:

  • To demonstrate the value of community-based care and thereby improve public understanding of the role of community-based care providers in society.
  • To present lectures, seminars, discussions on community-based care for provider staff training and development; consumer education; and knowledge and awareness for the general public.
  • To conduct research dealing with the technical, legal, administrative, economic, financial, and social health and quality aspects of community-based care services and to publish the findings of such research.
  • To prepare reports dealing with the legal, social, economic, technical, and other aspects of community-based care for the use of schools, educators, researchers, government officials and the public.
  • To promote, develop, and encourage innovative approaches to community-based care.
  • To recognize individuals and organizations for their outstanding contributions to the field of community-based care.
  • To identify gaps in community-base service delivery; to ensure a qualified workforce, promotes safety and standards of care, and promotes patient-center care and self-management.

What possible efforts does The Foundation For Community-Based Care, Inc. plan to undertake?

The purpose is to develop tools and resources that strengthen community-based health care and to obtain information and data that demonstrates the value of community-based care in modern healthcare delivery, and thereby improves the public’s understanding of the technical, legal, administrative, economic, financial, and social aspects of community-based care. Possible quality efforts may include but are not limited to such topics as follows:

  • Examine cost-effectiveness issues surrounding community-based care by compiling an economic impact study.
  • Develop a uniform curriculum for testing and validating direct care worker competency by offering a train-the-trainer RN certificate.
  • Investigate advancements in technology, such as telemedicine in its cost-effectiveness for use in an in-home or community-based care delivery system.
  • Identify discharge planners’ criteria for post-acute care placement when formal caregivers are available.
  • End-of-Life Care issues related to in-home and community based care options.
  • Legal, social economical, and ethical issues surrounding in-home and community-based care alternatives
  • Investigate the community-based labor pool shortages to identify possible initiative to increase retention of experienced staff and improve recruitment efforts.

Who are the Trustees of The Foundation For Community-Based Care, Inc.?

The Foundation is made up of 9 Trustees who bring a wealth of knowledge and years of experience regarding the in-home and community-based health care industry. This diverse group of individuals is linked by a common vision for in-home and community based programs. They strive to advance the goals of the Foundation by focusing on in-home and community-based health care needs.

Quality efforts and educational initiatives are funded through individual and corporate donations, special fundraising events, and private and public grants.
