Director, Clinical Services (Patient Service Manager III)

I. Primary Purpose of Position

The Director of Clinical Services (Patient Service Manager III) plans, organizes and directs clinical services to assure quality home care and appropriate quality of home health services for the University of North Carolina Hospitals. The Clinical Director-Patient Service Manager III adheres to federal, state and local laws, accreditation, policies and standards for compliance and reimbursable client care services and is responsible for the development of clinical policies and standards and ensuring compliance. Assumes responsibility for clinical operations of the Department on a 24 hour/day and seven-days/week basis.

II. Duties and Responsibilities

  1. Plans and implements all clinical services for the department through adherence to rules/regulations; interfacing with the other departments to coordinate clinical
    issues and clinical paths; and ensuring appropriate staff knowledge to achieve optimal clinical outcomes.
  2. Assists in the development of policies and procedures relating to home care. Uses continuous quality improvement activities to assure good clinical outcomes and involve all appropriate staff. Provides leadership by promoting and maintaining standards of quality care by all members of the client care team.
  3. Assists the Administrator with the development of an annual market assessment, budget, and business plan, including goals and objectives, and resource utilization. Monitors expenditures and adherence to performance measures through the implementation of controls. Participates in the review, analysis and evaluation of the effectiveness of the department.
  4. Directs the recruitment, hiring and development of the staff to ensure the delivery of safe and consistent quality care to all clients. Assures that clinical caregivers demonstrated the ability to perform accurate and complete assessments, communicate with physicians, plan for service delivery, plan for discharge and make good professional decisions.
  5. Completes staff performance reviews in a timely manner. Develops individual goals and objectives with staff to ensure their skills, knowledge base and growth needs. Coaches staff as appropriate.
  6. Collaborates with Nursing Supervisor – Patient Service Manager II to ensure appropriate visit frequency/duration and that all visits are appropriately covered. Assists with monitoring use of PRN and temporary nurses; ensures effective and efficient care on weekends and after hours.
  7. Directs coordination between client, intake, scheduling, care plan development and resource utilization. Establishes methods for coordination of care by all disciplines.
  8. Establishes/audits internal quality controls compliance issues associated with billing, payroll and records procedures associated with the scheduling and service delivery processes are timely, accurate and in compliance with policies.
  9. In collaboration with the Administrator, coordinates the Professional Advisory Committee and consults with the appropriate committee member in matters pertaining to client care services.
  10. Directs and participates in formal and informal interdisciplinary care coordination activities that effectively communicate to all appropriate persons regarding client problems, needs, psychological and spiritual concerns, and implementation of an individualized care plan of care. Consults with supervisors and staff, individually or in a group, regarding clients or families and special programs.
  11. Promotes staff development by: a) Utilizing capabilities of nursing and other professional personnel in the development of orientation and staff education programs; b) Recognizing leadership potential and offering opportunities for leadership training and development and c) Encouraging and assisting personnel to continue self-improvement through formal education, educational meetings and active participation in professional and related organizations.
  12. Supervises the Clinical Supervisors- Patient Service Manager II, Rehabilitation Supervisor, liaison staff.
  13. Serves as a member of the Professional Advisory Committee, Accreditation and Standards Committee, and other committees as assigned. Serves as the Chairperson for the Performance Improvement Committee.
  14. Participates in state and local organizations and activities related to home health professionals, community health services and agency services.
  15. Participates in and promotes cooperation between the university and community agencies to arrange educational experiences for students.
  16. Directs the research and development of new programs that satisfy community needs and disease management initiatives.
  17. Operates in a profitable and cost-effective manner by assisting with the preparation of the annual budget and adheres to budget parameters.
  18. Promotes an atmosphere of collaboration, participation and teamwork within the agency.
  19. Maintains an environment of safety for patients, staff, self and others.
  20. Develops short term and long term goals for agency and analyzes them in order to ascertain the extent in which goals are being met.
  21. Anticipate and respond to patients/customers/physician specific need and expectations with the appropriate sense of urgency.
  22. Listen actively and openly with understanding and sensitivity to others.
  23. Communicate in an open manner that encourages discussion and dialogue to resolve issues.
  24. Share information in an appropriate manner.
  25. Develop trust and good working relationship with all workers.
  26. Works with others to achieve team and organizational goals.
  27. Follows appropriate chain of command to resolve concerns with management decisions. Responds sensitivity to diversity of culture, opinion and approach among co-workers.
  28. Seek input and information from Administrator and provide constructive feedback on operations, process changes, and other organizational improvements.

III. Knowledge, Skills, and Abilities

Basic knowledge of governmental home health regulations, Medicare and Medicaid regulations is necessary. Good organization, communication, and human relations skills are required. Self-motivation, assertiveness and independent decision-making skills are essential.

IV. Minimum Education and Experience

Current licensure as a Registered Nurse in the State of North Carolina is required. Master’s degree in nursing, hospital or related field preferred. Four years of clinical experience to include two or more years in home health management/administration.
