• Demographic information such as the person's name, address, phone number, birth date, hospital admission and expected discharge dates, if applicable.
• The name and phone numbers of the physician who is caring for this
  individual.
• The primary diagnosis (medical condition and any other pertinent medical problems).
• Services, supplies, and equipment that may be needed or desired.
• Any special diet, medications, or treatments that the person requires.
• Availability of family or other caregivers to assist the person.
• Mental status and any psychosocial issues that could impact care
