Home care and hospice agencies will need a wide variety of information about the individual seeeking home care to enable them to assist in planning for needed services or by the patient or family includes:

• Demographic information such as the person's name, address, phone number, birth date, hospital adminssion and expexted discharge dates, if applicable.
• The name and phone numbers of the physician who is caring for this
  individual.
• The primary diagnosis (medical condition and any other pertinent medical problems).
• Services, supplies, and equipment tha t may be needed or desired.
• Any special diet, medications, or treatments that the person requires.
• Availablity of family or other carefivers to assist the person.
• Mental status and any psychosocial issues that could impact care
